Below is the most current Building Use Form. We have added a section for Zoom and hybrid meetings. Both a “regular” copy PDF for printing and a “fillable” copy will be posted on line so you can use which ever fits your needs (Instructions for using fillable form below).

NOTES FOR USING FILLABLE FORM:

• Before filling out the fillable form, to go to File and Select “Save As” and save to your computer, so and email to the CKA office once the form is completed. You can email the completed form in to office@kolamielmira.org or mail it to Congregation Kol Ami at 1008 W. Water St, Elmira NY, 14905.

• PLEASE NOTE:  If you use Adobe Reader for PDFs there will be a bar on the top right with tools. If you use Google Chrome the bar will not be there and you will need to print and finish by hand.

Please plan to complete the building use form and return it to the CKA office in a timely manner. Once it is completed and returned the office, it will be shared it with both the JCF office and with the kitchen staff, so everyone knows what is happening in the building.

 

The process should go like this:

• Anyone requesting to use the CKA building and/or a zoom account to check with the JCF office master calendar (in person or by email) and ask that their preferred date be held for three (3) days while they complete and submit a building use form.

• Once the Building Use/Zoom form is received in the CKA office & confirmed on the JCF master calendar: final copies would be given to the JCF Office and added to the kitchen staff book, if appropriate.

• The JCF office would put it on the master calendar and send email notification to CKA and the event responsible person

• (If applicable) the CKA or JCF office will email back the event organizer the appropriate zoom link so that the event users can share as appropriate with their group.
